Adenoacanthoma of the Stomach

Abstract
A case of adenoacanthoma of the stomach in a 43 year old Korean male is reported in view of the rareness of its kind and interesting aspects of the histogenesis.
Carcinoma with a squamous component originating in the stomach was first reported by Rolleston and Trevor in 1905. Since then about 43 cases have been reported in the literature, in which squamous cell element was present either alone or mixed with glandular component (adenoacanthoma).
Clinical complaints of the patient was postprandial epigastric pain associated with melena for about 4 months with other signs suggesting carcinoma of the stomach. Under the clinical diagnosis of carcinoma of the stomach, subtotal gastrectomy was performed.
Resected stomach showed a large oval shaped slightly elevated well demarcated lesion 9.2 x 6.3cm, with shallow central ulceration at the antrum of the lesser curvature, 5 cm above the pyloric ring. Histologically, a large area of central part of the lesion presented well differentiated squamous cell carcinoma surrounded by a narrow rim of relatively well differentiated adenocarcinoma at the peripheral portion of the lesion. Multiple regional lymph nodes showed metastases of only glandular element to superior gastric and celiac lymph nodes.
